PURPOSE OF POSITION:
Prepare patients for examinations, retrieve and update patient records and assist in the care of the patient.ESSENTIAL JOB FUNCTIONS:
JOB REQUIREMENTS:
- Review daily schedule and ensure needed paperwork is prepared and completed for each patient. Ensure medical and lab reports are available for the provider to review with the patient.
- Check in patients, assist them in the exam room, obtain and record vital signs, update medical history and medication record.
- Monitor immunization records for patients to ensure compliance with recommended immunization schedules. Enter immunizations into ALERT IIS.
- Assist provider during the patient examination and with special procedures, minor surgeries and other exams as needed.
- Under supervision of the provider and as qualified, provide direct patient care such as injections, immunizations, CLIA waived labs, catheter changes, ear irrigations, suture removal, cast removal, dressing changes, and basic wound care.
- Set up sterile trays and prepare rooms for procedures. Properly clean and sterilize equipment and instruments and dispose of contaminated items.
- Submit electronic charges for all nursing services performed at the provider’s direction.
- Communicate with providers, pharmacies, and agencies on behalf of the patient to ensure timely continuity of patient care.
- Under provider’s direction, follow up with patients to communicate test results or care instructions. Monitor and respond to patient requests via the patient portal.
- Clean and straighten exam rooms between patient visits and at the end of the day. Maintain work areas in a clean, orderly and secure manner. Make sure exam rooms are properly stocked with necessary supplies. Advise nursing supervisor when stocked supplies are low and need to be ordered.
- Cooperatively work with clinic staff, patients, other agencies, and the public to provide quality customer service courteously.
- Be strictly confidential about patient care and their medical record.
- Assist others in the clinic as workload demands.
- Knowledge of medical terminology.
- Knowledge of prescription medication classification.
- Knowledge of sterile technique principle.
- Knowledge of vital sign procedures and documentation.
- Knowledge of infection control procedures.
- Knowledge of English, grammar, spelling, data entry and retrieval, and computer applications software.
- Knowledge of math, medical terminology and health.
- Knowledge of confidentiality concepts and requirements.
- Knowledge of procedures for immunization, injection and medication administration
- Ability to communicate clearly with healthcare professionals, public, customers and staff in a courteous manner.
- High School Diploma or equivalent AND three (3) years of experience working in healthcare setting and in direct patient care
- Certified Clinical Medical Assistant (CCMA) (May consider EMT, CNA with experience)
- Certified in phlebotomy

- Assure appropriate coordination and communication of required feedback for insurance case managers, physicians and other authorized parties
- Assure patient and family needs are identified, prioritized, and met in order to achieve desired clinical outcomes.
- Utilize the principles of patient-centered, family-focused care in interactions with patients, families, physicians, and other care providers to identify agency strengths and weaknesses and intervene as necessary to provide for achieving desired patient outcomes.

- Collaborate with appropriate team members to develop and implement population-specific plans of care.
- Assist with the establishment, monitoring, and maintenance of a clinical documentation system that makes evident patient/family needs, interventions, and progress in accordance with clinical, quality, and regulatory requirements. Assure all documentation is available in the electronic record in an appropriate timeframe after the home visit.
- Complete initial and on-going comprehensive assessments of the patient’s needs/problems including OASIS assessments at the appropriate time intervals. Develop a care plan, which establishes goals based on nursing diagnosis and incorporates and initiates therapeutic, preventive, and rehabilitative nursing actions. Make necessary revisions as patient status and needs change. Regularly re-evaluate patient nursing needs on an ongoing basis and provides care which is consistent with the medical plan of care. Monitor patient response to nursing care provided. Observe changes in condition, reports such changes to the physician, and obtains orders for changes to the Plan of Treatment. Ensure physician orders are current. Administer medications and treatments as prescribed by the physician and in accordance with all federal and state laws and rules. Documents all observations, interventions, and physician orders in the clinical record.
- Observe and promote patient and family involvement in care planning, team conferences, and other mechanisms.
- Conduct regular team conferences with other home care disciplines to promote information exchange, determine care assignments, and to promote integration of care plan.
- Participate in field visits and conducts HHA Supervisory visits
- Assist with the development, analysis, and creation of clinical improvement opportunities through the use of quality and patient feedback systems; ensure incorporation of the results of, and techniques/principles associated with, scientific inquiry and quality management into professional practice.
- Maintain compliance with home care standards
- Serve as an effective liaison with local hospital, physicians, and other health care referral resources to facilitate referrals, continuity of care, and transition of care
- Maintain clinical records, statistics, and reports needed for purposes of evaluation and reporting.
- Report all client complaints and incidents; implement appropriate corrective action as directed and follow up when necessary.
- Participate in case conferences to promote more effective performance and delivery of care.
- Participate in quarterly chart reviews and communicate findings and recommendations.
- Submit an accurate record of visits/hours worked each day.
- Participate in taking call for agency on a rotating basis as assigned by the Director.
- Assist in orientation of all home care staff. Bring counseling concerns to the attention of the Director on a regular and ongoing basis, Validate service staff competency according to the agency plan prior to patient care assignment

PURPOSE OF POSITION:
The Physician Associate is credentialed and privileged in family practice and collaborates with thkelle multidisciplinary treatment team. He/she is able to perform advanced or specialized services and the performance of acts of medical diagnosis and treatment. Assessment, diagnosis, treatment plans and/or referral will be made in accordance with medical protocols on file and/or evidence-based standards of care as published in current literature. A supervising physician will be available to the physician associate for consultation, referral, and periodic chart reviews.ESSENTIAL JOB FUNCTIONS:
The following description of duties may not be all inclusive, and other duties may be assigned. Serve our patient population primary care and acute care needs. Support mission and goal achievement, organizational quality and patient-centered care. JOB REQUIREMENTS:
KNOWLEDGE, SKILLS AND ABILITIES:
- A minimum of one years’ experience is desirable. Active NCCPA certification from a AREPA accredited program preferred.
- Thorough knowledge of techniques and procedures, medical and lab equipment and testing procedures, medication.
- Considerable knowledge of motivational interviewing and counseling techniques.
- Ability to work independently, communicate effectively both verbally and in writing, deal tactfully and patiently with clients, establish and maintain records, reports and statistical data, use initiative and judgment, maintain effective working relationships with other related agencies, professionals and citizen groups, perform and record total physical examination, obtain vaginal, cervical, urethral and rectal cultures, and other laboratory specimens appropriate.

This is a volunteer position with the Harney County Sheriff's Office.
Purpose of Position
Perform duties and maintain custody and surveillance of prisoners confined in the facility. All duties related to operation of the jail and inmates. Does related work as required.
Essential Job Functions
- Maintain custody and surveillance of prisoners confined in the facility. Observe and report prisoner behavior and physical condition.
- Control prisoner movement within the facility. Supervise and instruct prisoners on work crew and on prisoner program assignments.
- Collect evidence and property, interview witnesses, and coordinate inmate disciplinary hearings.
- Inspect prisoner's quarters, issue items to inmates, i.e., clothing, personal hygiene items, meals, related utensils, and bedding. Maintain written and computer logs, including medication and care logs, of activities and prepare reports.
- Process persons in and out of the facility. Review documents of arrest and commitment. Fingerprint, photograph, and search prisoners. Prepare necessary paperwork and reports.
- Maintain jail security, control entry to and movement within the facility, escort and transport and provide security for court appearances, hospital, and other outside detention of prisoners.
