Salary/Benefits:         $33.02 to $49.18 (depending on years of experience) with full County benefits

PURPOSE OF POSITION:

Responsible for the direct line management of the patient care processes. Assists the Director with supervision/coordination of all aspects of the skilled nursing operations of Home Health Care as assigned by the Director.  Facilitates clinical management of patients receiving care. Assists with the development, implementation, and evaluation of referral systems and practices. Oversees all aspects of service delivery, including (but not limited to) patient referral, assignments, care plan development, ongoing patient care, and documentation. Assists with agency customer service and communications including physician communications. Participates in development of practice standards; including measurement and trending of patient outcomes related to care. Maintains services under pertinent federal and state requirements. Collaborates with the Director, peers, and clerical/financial staff to meet clinical, operational, and financial objectives.

ESSENTIAL JOB FUNCTIONS:

• Assist with the establishment, monitoring, and maintenance of an effective referral process between the agency and all referral sources.
• Address transfer of information, initial assessments/data collection, initial home care physician orders, patient and family education and preparation for home care, and provision of “seamless” integrated care with the attending physician and the referring organization.
• Assure appropriate coordination and communication of required feedback for insurance case managers, physicians and other authorized parties
• Assure patient and family needs are identified, prioritized, and met in order to achieve desired clinical outcomes.
• Utilize the principles of patient-centered, family-focused care in interactions with patients, families, physicians, and other care providers to identify agency strengths and weaknesses and intervene as necessary to provide for achieving desired patient outcomes.
• Deliver respectful care consistent with the patient/family’s home, cultural, and environmental circumstances.
• Collaborate with appropriate team members to develop and implement population-specific plans of care.
• Assist with the establishment, monitoring, and maintenance of a clinical documentation system that makes evident patient/family needs, interventions, and progress in accordance with clinical, quality, and regulatory requirements. Assure all documentation is available in the electronic record in an appropriate timeframe after the home visit.
• Complete initial and on-going comprehensive assessments of the patient’s needs/problems including OASIS assessments at the appropriate time intervals. Develop a care plan, which establishes goals based on nursing diagnosis and incorporates and initiates therapeutic, preventive, and rehabilitative nursing actions. Make necessary revisions as patient status and needs change. Regularly re-evaluate patient nursing needs on an ongoing basis and provides care which is consistent with the medical plan of care. Monitor patient response to nursing care provided.  Observe changes in condition, reports such changes to the physician, and obtains orders for changes to the Plan of Treatment. Ensure physician orders are current. Administer medications and treatments as prescribed by the physician and in accordance with all federal and state laws and rules. Documents all observations, interventions, and physician orders in the clinical record.
• Observe and promote patient and family involvement in care planning, team conferences, and other mechanisms.
• Conduct regular team conferences with other home care disciplines to promote information exchange, determine care assignments, and to promote integration of care plan.
• Participate in field visits and conducts HHA Supervisory visits
• Assist with the development, analysis, and creation of clinical improvement opportunities through the use of quality and patient feedback systems; ensure incorporation of the results of, and techniques/principles associated with, scientific inquiry and quality management into professional practice.
• Maintain compliance with home care standards
• Serve as an effective liaison with local hospital, physicians, and other health care referral resources to facilitate referrals, continuity of care, and transition of care
• Maintain clinical records, statistics, and reports needed for purposes of evaluation and reporting.
• Report all client complaints and incidents; implement appropriate corrective action as directed and follow up when necessary.
• Participate in case conferences to promote more effective performance and delivery of care.
• Participate in quarterly chart reviews and communicate findings and recommendations.
• Submit an accurate record of visits/hours worked each day.
• Participate in taking call for agency on a rotating basis as assigned by the Director.
• Assist in orientation of all home care staff. Bring counseling concerns to the attention of the Director on a regular and ongoing basis, Validate service staff competency according to the agency plan prior to patient care assignment
• Promote working unity between/among care providers, support staff, and consumers by creating an environment which promotes trust, risk-taking, and open communication.

 JOB REQUIREMENTS:

KNOWLEDGE, SKILLS AND ABILITIES

• Outstanding verbal and written communication skills aimed at a variety of constituents, patients, families, physicians, students and community groups
• Working knowledge of HIPAA privacy and security rules
• Ability to speak, read, and write the English language
• Excellent written and verbal communication and presentation skills
• Ability to collect, interpret, and/or analyze complex data and information
• Knowledge of nursing, hospital and home care systems
• Ability to willingly devote work time and effort in proactive and crisis management situations in an erratic/sometimes unpredictable work environment
• Must demonstrate the ability to practice independently in the delivery of nursing care
• Required proficient skill in the operation of personal computers. Keyboarding skill is essential and the ability to learn home health software application prior to completion of probationary period at time of employment,
• Ability to understand and appropriately respond to a diverse population and clinical responsibilities
• Ability to perform well in team environment, with staff at all levels, to achieve business goals
• Knowledge and skill in the use of general use of patient care equipment, including but not limited to: sphygmomanometer, oximeter, infusion pump, other
• Thorough knowledge of office practices and procedures
• Thorough knowledge of basic word processing software
• Basic knowledge of basic accounting and recordkeeping operations
• Basic understanding of medical terminology
• Knowledge of business English, composition, grammar and spelling

EDUCATION AND EXPERIENCE:

• Bachelors of science in Nursing from an accredited college or university AND at least 2 years nursing experience.

SPECIAL REQUIREMENTS/LICENSES:

• Current CPR certification for Healthcare Providers
• Current Oregon driver license, proof of acceptable driving record, an automobile that is reliable and insured as required by law
• Unrestricted license as a Registered Nurse in the State of Oregon
• Must learn home health software application(s) prior to completion of probationary period.
• Successful completion of appropriate background investigation(s) as pertaining to the position

PREFERRED QUALIFICATIONS:

• Experience in home health care delivery.
