Vital Records ~ Birth & Death Certificates

 

Birth & Death Certificates may be ordered at Harney County Health Department for all births and deaths occurring in Harney County within the past six (6) months.

Birth Certificates

Birth Certificates may be ordered at Harney County Health Department for all births in Harney County within the past six (6) months. Requests may be made in person. The fee for each certified copy is $25.

To order a birth certificate:

Death Certificates

Death Certificates can be ordered by eligible persons* at Harney County Health Services for all deaths occurring in Harney County within six (6) months of the date of death. Requests may be made in person. The fee for each certified copy is $25.

To order a death certificate:

After 6 Months

Certificates can be ordered up to 6 months from the date of the event.  After six (6) months have passed since a birth or death, the certificates must be ordered from the Center for Health Statistics office at 1-971-673-1190 or you may order at https://www.oregon.gov/oha/PH/BirthDeathCertificates/GetVitalRecords/Pages/index.aspx

Eligible Persons

*Access to death records filed in the State Vital Records office is restricted for 50 years after the date of the event to family members and their legal representatives, persons or organizations with a personal or property right, government agencies and persons licensed in Oregon under ORS 432.380. When ordering a death record, you must state your relationship to the registrant, or your reason for needing the record. If you are a legal representative of a family member, include whom you represent and how they are related to the person named on the record. If you have a personal or property right, explain why you need the record. If you are not eligible to order the record, please include a permission note with the notarized signature of an eligible person.
